- The applicant must be a permanent citizen of the country.
- Applicants must have a passport-size photo of their own.

Steps for One Nation One Health Card 2023 Apply Online
- First of all, you have to go to the official website of the National Digital Health Mission, click here to go to NDHM ‘s website.
- As soon as you go to the official website of NDHM, its home page will open in front of you.
- On the Home Page, you will see a link to Create Health ID and click on the link Create Health ID as soon as you click on the link of Create Health ID, a new page will open in front of you.
- Here you will see two options: Generate Via Aadhaar, Generate Via Mobile.
- If you want to create a Health ID Card through Aadhaar, then you have to select the Generate Via Aadhaar option, otherwise, if you want to create a Health ID card using a mobile number, then in this case you need to generate an ID via Mobile option.
- If you have selected the option of Generate Via Aadhaar, then you have to enter your Aadhaar number or if you have selected Generate Via Mobile then you will have to fill in your mobile number.
- Now an OTP will be sent to your mobile number, you will enter this OTP and as soon as you enter and submit OTP, the One Nation One Health ID Card Form will open in front of you.
